This is basically the frame of a steel cookie/baking sheet with the center of the sheet cut out so figures can be tied to it in the center via steel wire armatures. This frame is clamped and can be adjusted up or down by hardware on 4 vertical threaded rods. This vertical dimension defines the perceived distance between the mouth piece and the vagina piece. The mouth piece could sit on the same model of cookie/baking sheet painted black so the two frames could be almost snugly stacked over eachother. The drawing is truncated, not to scale, so there would probably be much more distance between the wooden frame holding a glass/transparent plastic sheet that holds up the lens and the closest subject.

Animators did a similar thing in Fantasia as technology was behind but their money wasn't. The audience can see this as different layers of trees leave the depth of field and get out of focus compared to the background. I have not tried anything like this before to see if it works well in an affordable manner.
